Dictation for nurses: charting, nursing notes and a Mac

Can nurses use dictation for charting? Why patient notes stay in the employer's EHR, what nursing bodies expect of records, and where a Mac dictation app helps.

Nursing charting happens in your employer's electronic health record (EHR), on the employer's devices, under the employer's policy, using any speech recognition the employer provides there. A dictation app on your personal Mac is for writing with no patient information: study notes, coursework, emails, reflective writing, handover templates. Using it for anything about real patients needs your employer's explicit approval, because a compliance decision belongs to your organisation, not to you or to the app.

This page is general information, not legal or clinical advice.

Why does charting stay in the employer's system?

Nursing bodies treat the record as part of practice, and they tie it to the organisation's systems and rules.

  • The American Nurses Association's Principles for Nursing Documentation (2010) says "the nurse must be familiar with all organizational policies and procedures related to documentation and apply these as part of nursing practice." It names the EHR as the foremost electronic record and asks that entries be "accurate, valid, and complete," "authenticated," and "dated and time-stamped by the persons who created the entry."
  • The same document says protection systems must be built into documentation systems for "security of data," "protection of patient identification" and "confidentiality of patient information."
  • In the UK, the Nursing and Midwifery Council's Code, section 10, asks nurses to "complete all records at the time or as soon as possible after an event," to "attribute any entries you make in any paper or electronic records to yourself," and to "take all steps to make sure that all records are kept securely." Section 5 asks you to "respect a person's right to privacy in all aspects of their care."

A personal dictation app sits outside all of that. It does not sign your entry, timestamp it in the record, or sit behind the hospital's access controls. Text you dictate on a personal Mac may also be kept in the app's history, the clipboard, a notes app that syncs, or a backup the employer does not control.

What about HIPAA?

In the United States, HIPAA applies to covered entities such as health care providers that submit claims electronically, as the Centers for Medicare & Medicaid Services explains. Your employer is usually one, and its HIPAA policies govern how you handle patient information. If a dictation service receives patient information, the employer would normally need a business associate agreement with that service. Our medical dictation and HIPAA guide explains that in more detail.

Removing a patient's name is not the same as removing identifying information. HIPAA's de-identification rule, 45 CFR 164.514(b)(2), lists identifiers that have to go, including names, dates other than the year, medical record numbers and "any other unique identifying number, characteristic, or code." A handover note that says "bed 12, admitted Tuesday, the one with the rare transplant history" can still point to one person. Treat any note about a real patient as patient information.

How do you dictate on a Mac?

The free option is built in. Apple's Dictation guide (macOS 27 edition) says to choose Apple menu → System Settings, click Keyboard, and turn on Dictation. Then click where you want to write and press the Microphone key or your Dictation shortcut. Apple says Dictation automatically inserts commas, periods and question marks in supported languages and stops after 30 seconds without speech. Full steps are in How to use voice to text on a Mac.

Clinical terms, drug names and abbreviations are where general dictation struggles most. Read every line. A misheard dose or a dropped "not" is easy to miss because the wrong word is spelled correctly. Improve Mac dictation accuracy covers microphone, noise and vocabulary fixes.

Where does SuprFlow fit?

SuprFlow is a general Mac dictation app, not clinical software. It holds no certification, offers no business associate agreement, has no EHR integration and makes no promise about medical vocabulary. Do not use it for patient information unless your employer has approved it.

For non-patient writing, it works like this on an Apple Silicon Mac with macOS 14 or later: hold the Fn key, speak, release, and the text is pasted at your cursor. Speech is recognised on the Mac with a downloaded model, Default formatting is local cleanup of punctuation, capitalization and numbers, and a personal dictionary can hold course or ward vocabulary. It keeps a local history of up to 100 recent transcriptions, and it uses the network for licence checks, updates, model downloads and usage telemetry. It costs $125 once, with no free tier or trial. For study notes and emails, Apple Dictation is free and may be all you need.

Questions people ask

Can nurses use voice to text for charting?

Only through the systems and tools your employer provides and approves. If your employer offers speech recognition inside its record system, ask for training on it. A personal app on a personal device is not part of the record system.

Is it OK to dictate a handover note at home and paste it in later?

Not with real patient details, unless your employer explicitly allows it. The note would sit on a personal device, outside the record, and the NMC Code asks that records be made at the time or as soon as possible after the event.

What is the best dictation software for nursing students?

For coursework, start with the free Dictation on your Mac. Use invented patients in any practice care plan. If you need adjustments because of a disability, arrange them through your school's disability service.

Can I dictate my nursing revalidation or portfolio reflections?

You can dictate them, as long as no patient can be identified. Check the anonymisation guidance from your regulator or school before you start.
